Survey templates are pre-built surveys that come with questions, answer options, and recommended channel visibility settings already set up for you. They're designed around common goals that merchants like you have, so you can get a survey live in just a few clicks.
How to use a template
Click the Create survey button on your Grapevine Surveys dashboard.
Select the Use a template option from the dropdown.
Browse the templates and use the category tabs at the top to filter by goal.
Click View questions on any template to preview the questions it includes.
When you've found one you like, click Use this template.
That's it!
The template will load into the survey editor with all the questions, answer options, and channel settings pre-filled. You'll see a confirmation message letting you know everything has been preloaded.
What's included in a template?
Each template comes preloaded with:
Questions and answer options - a curated set of questions using the most appropriate question types for that survey's goal (single choice, multiple choice, open text, NPS, CSAT, and more).
Channel visibility - recommended channels are automatically enabled (for example, Checkout app block for post-purchase surveys, or Standalone page for surveys you'll share via email or links).
Display rules - for post-purchase surveys, templates include targeting rules such as showing the survey only to returning customers.

Instant Feedback
Surveys designed to capture quick, in-the-moment feedback from your customers.
Acquisition Insight Survey - Find out where your new customers are coming from and what influenced them to buy. Includes 4 questions. Channels: Checkout app block (new customers).
Checkout Experience Survey (CRO) - Measure how well your checkout process is working and identify friction points. Includes 2 questions. Channel: Checkout app block.
Lost Sales
Surveys focused on understanding why potential customers didn't complete their purchase.
Conversion Friction Survey (Abandoned Cart) - Find out why shoppers left without completing their order, so you can remove barriers and recover more revenue. Includes 2 questions. Channels: Standalone page, One click.
Customer Sentiment
Surveys that help you measure loyalty, satisfaction, and overall brand perception.
Delivery & Loyalty Survey (NPS) - Track how loyal customers feel after receiving their order. Includes 3 questions (NPS scale, open text follow-up, and a recommendation score). Channels: One click, Standalone page.
Returning Customer Insight Survey - Learn why customers come back and what they value most about your brand. Includes 4 questions (multiple choice, open text, CSAT rating, and open text). Channels: Checkout app block (returning customers).
Further Insights
Surveys for digging deeper into specific areas of your business.
In-Store Experience Survey — Monitor how customers rate their in-store experience and what brought them there. Includes 3 questions. Channel: POS.
Customising a template
Once a template is loaded, it's yours to edit. You can then:
Add, remove, or reorder questions.
Change the question text or answer options.
Adjust channel visibility and display rules.
Rename the survey.
When you're happy with your changes, give the survey a title related to the feedback or insights you are hoping to collect and click Save to make it live on your selected channels.
Templates are just a starting point - think of them as a recommended foundation that you can shape to fit your store's specific needs.
